Consent and Release Form
Without expectation of compensation or other remuneration, now or in the future, I hereby
give my consent to Hive to use my image and likeness and/or any testimonial statements
from me in its publications, advertising or other media activities (including the
Internet).
This consent includes, but is not limited to:
- Permission to use my name; and
- Permission to use quotes from my testimonial(s) (or excerpts of such quotes),
photograph(s), or reproduction(s) of me, in part or in whole
I, , agree to the terms of this consent and release form and I agree that my typed
name below can be used as a digital representation of my signature to that fact.
